I need the right information for a plugin i have installed?

I have a plugin and need the right information to fill in the blanks.
Here it is asking for? I know my user name and password which I will fill in to show you, in case you can find out for me please.

Password [redacted]
The next two below I have put what I thought it was but when I test it with the plugin it is not accepting the details. Could you please let me know so I can update the plugin.

First, you should NEVER post your username and password in this forum. This forum is PUBLICLY VISIBLE, and you just told the world how to log into your site.
I’ve flagged this for a Staff member to redact the username and password, but I’d suggest changing your password immediately, so that nobody else can log in.
