I need this blog removed from my wordpress.com account

  I had this site ( revelationmag.com ) linked with jetpack from wordpress self hosted blog but it no longer belongs to me so i need it gone from my account on wordpress.com.

  Please help.

  Would do it my self but found no option for it, i already unlinked from revelationmag.com admin but it stills comes out here.
