I noticed that the price for the domain is $18. Is that $18/year? What all does that come with? Just the domain or does it come with a website builder as well?

Yes, it is for a year and has to be renewed annually. No there is no website builder here. That is a reference to a sowrdpress.ORG software install on paid hosting.
if you purchase a domain mapping upgrade all that changes is the URLs, which will seamlessly redirect all visitors from the old URLs to the same content under the new domain URLs.

You may want a customized domain name. For example, YOURBLOG.com

Domain mapping of an existing domain URL that you already own and purchased elsewhere is not done free of charge. You will have to renew the domain name purchase where you made, it and the domain mapping purchase here, each and every year.
Mapping an existing domain costs $13. annually and a domain name purchase and domain mapping are included in the Premium upgrade bundle.
In order to map to a domain one must have an underlying .wordpress.com sub-domain blog to map from and it does not matter what that underlying .wordpress.com URL is. It does not have to match the domain URL. If you do not register a .wordpress.com blog URL first then the software supplies the dot whatever URL so you can proceed with your domain mapping purchase.

I did. This is saying it is $99/year. Before it said it was $18/year. It also does not say what all is included when you pay the $18/year.

Domain owners are required to share their contact information. The $8.00 Private Registration upgrade will protect your identity and hide your contact information.
Adding a domain you already own costs $13.00 per domain, per year.

To design the website on WordPress it is an additional $13/year? $18/year is just for the name.
You cannot do any CSS editing unless your purchase either the Premium or Business upgrade as both include the Custom design upgrade in them.

This does not include any privacy protection.
Domain privacy is something that is done through your domain registrar. So if you already have a domain name registered elsewhere, then you would purchase domain privacy from your registrar, not WordPress.com.
If you register a domain at WordPress.com ($18/yr) there is an option to add Domain Privacy to that domain registration for an additional $8/yr.

Using WordPress.com is completely free to have a _____.wordpress.com URL.
If you have a domain registered elsewhere and wish to use it with WordPress.com, you need a $13/yr upgrade called Domain Mapping.
If you Do not own a domain yet and want to register one, you can do so with WordPress.com for $18/yr. You do not need to purchase Domain Mapping for a domain that is registered at WordPress.com. So the charge is just the $18/yr, not $18 +$13.
$99/yr is for WordPress.com Premium. Premium is a bundle of upgrades that includes, among other things, a free domain registration.
