  How do I add a Signup Form to my Blog so I
  can maintain a List of Viewers?

  Compelling visitor registration is not possible on a free hosted WordPress.com blog. If you want to requires registration you will need to hire a web host and set up a WordPress.org install. http://support.wordpress.com/com-vs-org/
