What does the $18 option include? Also, why, if you must chose another option during the next setup step, is there not an option that is free of charge?

Domains are no longer available for purchase or mapping individually. They are included with WordPress.com Premium. This change has been made as we work to offer the best combination of value and performance for users while still offering a completely free product for those who do not wish to purchase any upgrades.

If you want the custom domain but aren’t interested in a premium WordPress.com plan, you may want to consider a self-hosted (WordPress.org) WordPress site.
