I would like to add documents to my webpage as a link to fill out a form

  Hi,

  I am having a few problems in regards to getting a word document to attach to the webpage as a link to a document. I am happy to convert to PDF if that is easier, but I am not sure if the website has the capability to do this?

  Also, when I add a contact form on my contact us page, it fills in the name with our user name to log into wordpress and the email comes up with our email… is there any way to have these field left blank for people to fill in for themselves.?

  It is possible to upload documents straight to your site following the instructions here:

  Upload a Document

  However, it’s only possible to link the item for download. The document wouldn’t be embedded for visitors to fill out. Does that make sense? The visitors would be able to download the form, but not submit changes back through your site.

  The form is being pre-populated with your contact details because you’re logged into WordPress.com. However, visitors will either A) see their own details if they’re logged in or B) see empty fields. You can test this by logging out and visiting the contact page.
