Images,Elementor and admin pages return 404 or fail to load on the first attemt

Hi everyone,
I’m facing an issue on my WordPress site where several elements fail to load properly on the first attempt but work fine after a refresh.Observed issues:
- Some images show a 404 error on the first load, but appear correctly after refreshing.
- The Elementor editor doesn’t load initially (stuck on loading screen), but loads fine after the second attempt.
- Some admin pages (like
/wp-admin/post.php) intermittently return 404. - A few frontend pages also occasionally show 404 errors.
Environment:
- Server: Apache on Linux
- PHP: 8.0.30
- MySQL: 5.7.44
- Memory Limit: 256M
- GD / ZIP: Installed
- No caching plugin is used
Findings so far:
- The System Info reports a write permission issue with the
.htaccessfile. - There’s another
.htaccessfile inside/wp-content/, created by the Converter for Media plugin (handles WebP/AVIF conversions). - If I open the image link directly in a new tab, it loads normally, meaning the file exists.
- This issue affects Elementor, images, and admin URLs inconsistently.
What I’ve tried:
- Regenerated
.htaccessby re-saving Permalink Settings. - Temporarily disabled the
/wp-content/.htaccess(media converter rules). - Verified that mod_rewrite is enabled.
- No caching or CDN is used.
- Checked file permissions and found
.htaccessmay not be writable by WordPress.
Current situation:
The issue still persists — Elementor and images fail to load on the first attempt, but everything works fine after a refresh. Request for help:

Could conflicting rewrite rules or.htaccesspermission issues be causing delayed routing or missing headers that affect Elementor and media loading?Any advice or guidance would be greatly appreciated.
