  I need to transfer my followers of http://www.prateeksachan.wordpress.com
  to my new self-hosted http://prateeksachan.com/a/wordpress/

  (yes, i’ve already installed Jetpack)

  Hi there,

  I’d be happy to help you with this. However, I see that Jetpack still has your old development URL for your self-hosted site instead of the correct URL. Can you please disconnect Jetpack from your WordPress.com account and then reconnect it?

  That will refresh the URL in our system and make sure your followers are transferred to the right place. :)
