Information submission form including image upload

  Hi all,

  Ideally I am looking to create an information submission form for businesses on my website whereby a page will be created for them and they must be able to upload image of their logo.

  I have installed Buddypress but it seems complicated and I know that might seem weird to some of you – but I’m referring to “contact form 7” for example, that is so simple with a shortcode.

  Basically I would like an editable form even if it doesn’t have shortcode (but preferably does!) I can simply ad it to a page like contact form 7 and info will be sent to database. Remember it must include option to upload an image.

  You can’t create any forms on WordPress.com.

  Seems like a self-hosted WordPress.org based site suits to your requirements.
