How do I create a book now button that links to an online form to be filled in by the client?

You seem to already have a “book now” link in your menu that takes you to a page with a contact form on it. Are you still looking for help or is this now resolved?

I have been playing around with the normal contact form on my book now page, but it is not comprehensive enough for what I need.
I would like the Book now button to link to a much more in depth form that I could create myself?
Also, could there be a calendar button that they could choose which dates they would like to book the appointment for?

The built in contact form doesn’t offer that level of customisation. You could add drop downs for day, month and year but there’s nothing stopping a user from picking the 31st of September or any other day that doesn’t exist.
You could set up a form/survey somewhere else (I’ve seen people using SurveyMonkey quite a bit, but I’ve not used them myself) and see if they offer the extra options you need – you can still include it with an external link in your custom menu.

I am trying to put a 30 second video banner into my header image. But I cannot go any further than the crop and select page? Am I doing something wrong?

In Gateway you specify the video on the Theme Options page of the customiser. The full instructions can be found about half way down the Gateway theme page. Try working through the steps there and if you’re still not getting anywhere let me know where you get stuck.
