Installed new operating system after crash and now getting wordpress set back up

  I recently had to reinstall a new operating system after crash, so I upgraded operating system to windows 8, now I am trying to reinstall wordpress 2.9.1 which is the version my website http://www.jwflyfishing.com was built on. I downloaded wordpress 2.9.1 for archives, but now I am stuck with these instructions and where to find this information from my server. Any help would be much obliged. Bluehost is the server. What is confusing me here is that where you put the DB name when I look at bluehost info I have 3 separate DB names, so how do you put all three in there? Here are the instructions:
