Is my own information supposed to appear in the "contact form"?

  Hi, I’ve just made a page for my contact form, which I assumed was a place for readers to fill out their own name and info and send me a message.

  But I see that the form is automatically filled out with my own name and contact info. Have I misunderstood what this form is for? How can I create a contact form that is blank?
