The blog I need help with is: (visible only to logged in users)

Be aware that harvesting email addresses without express prior permission being obtained is a legal issue in countries like mine ie. Canada.
Create a static page > http://en.support.wordpress.com/pages/ and specify exactly why you want to harvest email addresses and state clearly that anyone who uses the form is consenting to that email address harvesting.
Place a contact form on it > http://en.support.wordpress.com/contact-form/
Create a custom menu http://en.support.wordpress.com/menus/
