Jetpack Contact Block-Display Issue

  I would like the form to not autofill in my name and my email for all to see. I would like it to display either a blank name and email, or whatever I want. I find by it displaying my name and email, it makes it confusing for where they are supposed to enter their name and email.
  I tried to edit the code but it has no effect.
