List members/fans of my blog

Hi,
I would like a way of people being able to become sort of a fan of my blog and then list them on a page.I am basically travelling the world for charity and need people to express an interest to meet me for a drink.
So I need in a form:Name, City, Location, email
Then list them all except for the email on a page.
Sorry if I have posted in the wrong place.
