Looking for a scheduling/reminder plugin

  Hello.

  My client would like to add a function to their website whereby a visitor would fill in some question with regards to when certain things would have to be done at their property. And then a week before the due date a reminder email is sent out.

  So for example the questions may be:
  1: When is your gas policy due? {add date}
  2: When is your building insurance due? {add date}

  And then a week or so before the added date a reminder email is sent.

  Does anyone know of a plugin that will help me do this?
