Looking for donation plugin

  I’m looking for a plugin that will allow people to give money and leave a note with it.

  This is exactly what I’m trying to do: http://store.indigosilverstudio.com/wedding-registry/25-registry-credit?imageID=0

  I’d like to be able to add a text field with a product. Basically, have buyers be able to leave me a note and have that note follow the purchase so I can know which buyer left the note.

  Anyone know of of a plugin that will give me this option?
