Looking for Theme With Three Search Fields

  Hello,
  I am making a new blog and am looking for a template with three search fields/boxes. For example “name” “city” and “date” could be entered into each box to get an answer to the client.
  Is there anything out there like that?

  I could be wrong, but I do not think there is anything on WordPress.com along those lines.
