Lost backup codes for account

I have a WordPress account under the email (email visible only to moderators and staff) that I am unable to access as the codes on my Google Authenticator aren’t being accepted, and I can’t seem to find my backup codes.
Help please. :(

I tried to fill out that form, but it said it couldn’t submit because my blog wasn’t hosted on WordPress.com. It’s not, I host it through DreamHost, but my account is still a WordPress.com account, I can no longer access it and other services such as Gravatar and Akismet. Access to the later is especially important as I’m subscribed to their service and can no longer access my auto payment data.
