Lost my user name & password!

I’ve forgotten my admin user name and password for my New Moon Arts blog! I need to edit it, asap. I have the admin URL: http://newmoonarts.wordpress.com/
How can I find the info I need to access and edit this WP blog, as administrater?
PS: this is NOT about the URL listed in the “Select the blog you need help with:”, above. The New Moon Arts blog was not an option in the pull-down menu, for some reason.
The blog I need help with is: (visible only to logged in users)

When we create a username account at WordPress.com we provide an email address and it becomes our unique identifier. Every blog we register when we are logged in under that username is registered to that username.
If we use this process to choose a different display name then it will appear everywhere, except here in the forums. As you cannot recall your log-in information I recommend that you try clicking this link:
http://newmoonarts.wordpress.com/wp-admin/
enter the display name if you created one or enter newmoonarts
then use the lost password link http://en.support.wordpress.com/passwords/#lost-password
