Main 'My Site' page, blog icon near blog URL is a grey world icon.

  I followed the steps to add a ‘blavatar’ and uploaded my icon.
  When I hover over the ‘My Site’ from my blog page, I can see the icon, but I can’t see it on this main page. Instead, it is still the default grey world.

  This is also the case when viewing my blog posts from Reader. Only the world is shown, not my icon.

  Do you know why?

  Hi! Actually I’m seeing the cool green cat icon. Your browser is probably just stuck remembering that grey world icon.

  Try loading up this page:
  rosemills.wordpress.com/favicon.ico

  If you don’t see the green cat there, let me know. Otherwise, next time you view your site you should see the right icon. I hope that helps!
