Make the submit button work?

Hi everybody,
On the footer of the home and core pages of http://www.miamiweddingsandevents.com we have a newsletter signup. It’s in there as a table, but no code to actually submit it. Is there a plugin that will help with this, or can somebody help me out with where to locate the code to make this work? I’m just now beginning to work on the site, and I worry how many potential contacts have been lost with this not working.
The existing code is as follows
