Making a submit button

Ok this is what im trying to do.
Im making a quiz for my blog. In the quiz it will be 5 questions. When they answer the questions in a comment box everyone can see the answers. Well I odnt want that. I want the people to submit the answers to either my e-mail or onto a page that only i can view. The problem is i have never had luck with submit buttons. So how do i make one and how do i link it to other pages or my e-mail? -
My suggestion would be for you to make a “contact page” which you can name anything you would like and have them submit their answers there. The submit button could be linked to the contact page so that it would take them there so they could send in their answers automatically to your email.
As far as a submit button, you will have to make an image you like, and then upload it to your blog from the upload section in the write page area.
Image links must be inserted into the post or the page in “code view” and would look like this:
