Media file disappeared?

  I changed my theme today and added color. Now one of the pictures I’d uploaded has disappeared. I tried re-loading it but I just get a question mark in the middle of the space.
  What happened?

  A few days ago some users reported issues with images not displaying properly, but that seems to be resolved.
