  In the process of updating the menu, I erased CONTACT and am unable to obtain the original template, for example..Name (required)
  Email (required)
  Comment (required)
  SUBMIT

  The blog I need help with is: (visible only to logged in users)

  Hi there,

  If you’ve deleted the contact page, you can bring it back by following these instructions –
  https://en.support.wordpress.com/forms/contact-form/

  If you’ve deleted the ‘menu item’ named ‘contact’ but not the actual contact ‘page’, then you simply have to add the menu item back. More info here –
