Migrate Followers from WordPress.com to a Self-Hosting Site

Hi, I’m switching to a self-hosted site and want to know if it is possible to keep my followers when I move.
My old blog is http://wealththrunutrition.wordpress.com, and I am moving to http://blog.wealththrunutrition.com. Apologies if this is in the wrong place. Thanks for your help.
The blog I need help with is: (visible only to logged in users)

Go ahead and install and activate Jetpack on your self-hosted site. Then connect it to WordPress.com.
A staff member will then be able to help you move your subscribers.

Hi there,
I’d be happy to move your followers for you. :) To make sure that all of your followers end up in the right place, can you please disconnect Jetpack from your WordPress.com account and then reconnect it? Then I’ll be able to move them over.
After I move them, your email followers will continue to receive email updates, and your WordPress.com followers will continue to see your new posts in the Reader. If your WordPress.com followers would like to receive email updates, as well, they can subscribe via email on your new blog.

Thank you for the help. I’m assuming deactivating/activate is the same as disconnect/reconnect for the Jetpack plugin. If so, then Jetpack is all set.
