Migrating blog to self-hosted version of WordPress

We are migrating the blog completely from WordPress.com to a self-hosted version of WordPress (.org). We would like to forward the URL to the new blog address, and when the custom domain expires in November, we plan to purchase it on our existing hosting account.
In the meantime, we would like to change the nameservers for the custom domain so that we can redirect the old WordPress.com urls using htaccess on our existing server. Where can I change the nameservers on the custom domain?
Is there a way to easily transfer followers, or will that service expire as well?

Provided you have installed and activated the JetPack plugin http://jetpack.me on your WordPress.org install, and post the relevant URL(s) into a forum thread, Staff will do the transfer of both subscribers and stats for you.
Please note that any WordPress.com followers who opted to receive email updates will need to re-subscribe again on your new site through the Jetpack signup box, otherwise they’ll only see new posts in their Reader. Non-WordPress.com email subscribers will continue to get email updates.
