Migrating followers from one self hosted site to another.

I recently had to change my website due to a few issues with the previous one. However, both sites were self-hosted and I am trying to figure out how to transfer followers from abytch.com to k-woo.com. I looked up JetPack directions but they seem to only pertain to wordpress sites to self hosted sites. Please assist and thank you in advance!!!
