Migrating Followers to Self Hosted

I recently switched from wordpress.com to wordpress.org and self hosted with blue host. I have the jetpack plug in but none of my followers seem to transfer over to my new site. I have tried reading forums on how to transfer them but nothing seems to be working. What should I do?

I installed the jetpack a few days ago when i was creating my new blog and was looking at the directions but I have a problem when I use the subscription migration tool steps and they tell you to log on to wordpress.com and go to settings and then go to ‘migrate followers’ on wordpress.com I do not have a site that says MyJetpackSite.me so maybe I installed it wrong… my URL to my wordpress.com site is
http://www.huntershealthykitchen.wordpress.com but it forwards you to my new site. -
