Migrating my followers from a wordpress hosted domain to another hosting company

  Hi,

  I just transferred my blog from WordPress to a different hosting company. I had purchased the domain through WordPress. My domain on WordPress was: http://www.brickandvine.com. I now host that domain myself. Is there a way to bring my followers with me? I have JetPack installed.
