Mimic Hard Copy Forms To Print And Collect Data

  I have a hard copy form I would like to post on my website I am creating in WordPress. I would like the form on uploaded to WordPress to look exactly like the hard copy.

  I want the form fillable and also printable so they can sign. When they click print, I would also like to collect the data they entered. Is there a way to do this? Thanks.

  I don’t know about a form you made on your own, but Wufoo and JotForm are options for your WordPress site. More information about them can be found here:

  Wufoo looks like the option closer to what you are looking for.
