modlook – transfer WP.com subscribers to WP.org self hosted site

  We have just started a self hosted site using WordPress. The site is maggiemoosetracks.com. How can we go about keeping our subscribers on the new site? We have activated and linked using JetPack and redirected site to the self hosted url. Can you help?
