move subscribers

  I need someone from WordPress to help move my subscribers from my WordPress hosted site to my self-hosted site. Original blog is lifetreetherapy.wodpress.com and new one is http://www.lifetreetherapy.com. I installed Jetpack and activated subscription modules as the instructions informed me to do, but can’t find a way to contact staff directly as is the next instruction. Thank-you!!
