move subscribers from wordpress.com to wordpress.org

  Hi there,
  I have moved my blog to a self-hosted site, installed Jetpack and enabled subscriptions and posted to past subscribers to follow on my new site. But the forum article said to contact you to move over my wordpress.com followers to my new site.
  The url is http://www.lifewithlampnsofa.com for both wordpress.com and wordpress.org.

  The blog I need help with is: (visible only to logged in users)
