Move subscribers to self hosted site

  I want to transfer the subscribers from http://legacytrackercanada.wordpress.com to the self hosted site http://legacytracker.com. Jetpack is installed at the self hosted site and the blog subscriptions widget is activated.

  The blog I need help with is: (visible only to logged in users)
