Hi there. If you already have a domain and hosting you’re off to a good start. Next you need to download and install the WordPress.org software and a theme to that domain. For assistance with that you’ll need to post in the WordPress.org forums as we can’t help you with that part here: http://wordpress.org/support/ (If you don’t already have a username on that forum, click the register button in the upper right corner so you can post in that forum.)
Once you have that site up and running you can then transfer your content from your wp.com site to the wp.org site by following the instructions here: http://en.support.wordpress.com/moving-a-blog/#moving-to-wordpress-org
To move your subscribers over to the new blog you’ll have to install the Jetpack plugin. Instructions here: http://en.support.wordpress.com/moving-a-blog/moving-your-subscribers/#wordpress-com-to-self-hosted-site
You can also have staff do this whole process for you by purchasing a Guided Transfer. See: http://support.wordpress.com/guided-transfer/
Lastly you might want to consider setting up a Site Redirect so visitors to your wp.com site automatically get redirected to your new self-hosted site: http://support.wordpress.com/site-redirect/
