Moving email subscribers

  Hello, I have recently moved to a self hosted site, and I would like to move my email subscribers to my new website. I have installed Jetpack. Can you help me with this? My old WordPress address is geraniumseeds.wordpress.com, and my new address is http:/prayersandchocolate.com.
