Moving followers to a self hosted blog with Jetpack

Hi,
I’m about to move my wp.com blog to a wp.org.
I saw in other threads that when you want to keep your followers, you have to download and install the Jetpack plugin, and provide them the two URLs of your old and new blog so that Jetpack engineers can move them.
In my case, the two URLs are the same as I already used my own domain -registered at wordpress.com – and as I will of course keep it for the new self hosted blog. I never used the mydomain.wordpress.com address. Will it work too ?
Also, I was wondering if Jetpack will move both email subscribers AND worpdress followers ?
And is this something that has to be done at the same time or AFTER the migration ?Thank you !
