Moving followers to self-hosted site

  I recently moved from wordpress.com to a self-hosted wordpress.org site and would like to move my followers to the new site.

  Staff cannot do this until you:
  install and activate Jetpack;
  provide the URL for the domain;
  provide the URL for the .wordpress.com blog
  Please do those now.

  I’d like to have my followers moved to my new domain…
  (1) Jetpack is already active
  (2) the new domain is http://www.smith-wellness.com/memoirs
  (3) the current domain is chosen2k7.wordpress.com
