  I want to move my blog out of wordpress to another host and create a custom domain name. What do I need to do in wordpress to get this done?

  The blog I need help with is: (visible only to logged in users)

  I see that http://climbthewell.wordpress.com/ is redirecting to https://climbthewell.com/

  Seems you have upgraded to a paid plan at WordPress.com and added a custom domain name to your site. Let us know if you need further help.
