Moving my wordpress.com subscribers to my self hosted site

Hello, you said we could ask for hep to move our subscribers to our self hosted site, please kindly help me!
I’ve install the Jetpack plugin on my self hosted site, connected it to WordPress.com with my WordPress.com username, and made sure the Subscriptions module is activated. My wordpress site is perryztot.wordpress.com while my self hosted site is perrystots.com
