This can be done by the WordPress.com support team by simply contacting them at http://support.wordpress.com/contact/ with the details you’ve provided here. Shoot them a message and they’ll have your subscribers moved over very soon!

It looks like you figured out you can actually do this on your own, as I can see your subscribers on the new site now. But let me know if you still need any help.
If owners of self-hosted sites need help transferring subscribers, you can point them to these instructions:
Also, rather than pointing people to the contact form, please add the tag “modlook” to the thread instead. Users who don’t own upgrades will only be brought back to the forum if they submit the contact form, and in fact this thread itself was created via the form, as indicated by the “wpcomhelp” tag in the sidebar.
