Moving subscribers from wordpress.com to self-hosted blog

I need to move email subscribers from wordpress.com blog to a self hosted blog.
I have already installed the jetpack subscribers module.
The wordpress.com website is still located at:
mommyhoodunplugged.wordpress.com
The self-hosted website is http://www.babysideburns.com
The blog I need help with is: (visible only to logged in users)
