my contact form disappear whenever i type in the box

  i am a newbie at wordpress and i have put a contact form in my blog but anytime i click on any of the boxes, the page disappears.
  Please help me out?

  I’ve gone ahead and taken a look at your blog, and I’m able to use your contact form without a problem. For reference, here’s the contact form I tested:
