My own information shows up on my contact form..

  Hi, I set up a contact form on my site https://adaptiveenvironment.wordpress.com/, but my own name and blog address, etc show up in the info boxes. How can I change this to make the boxes blank? Thanks.

  The blog I need help with is: (visible only to logged in users)

  These contact forms auto-fill when you are logged into your WP site. When I look at your contact form, my info is included. If you log out, and check the form, it should be blank.

  No one else will see your contact information.
