My sub-menus not showing under Categories

    My Categories are in my Primary Menu which is the heading on my opening page and each category is a country, the sub-menu being cities and towns within that country that I write about. Only some appear in this sub-menu when I, or a reader, clicks on these categories and the others don’t appear at all. What am I doing wrong and how can I correct this?
