Need a custom user registration page for manual approval

I have tried several plug-ins that allow for manual admin approval, but none of them let me customize the initial registration page. We are a wholesaler, and we want to gather the shop information before approval so we don’t have to constantly email everyone back asking for it so we can verify who they are. Anyone know of a plug-in that will do this, or any idea what might work for us?

it currently has a force-login plug-in as well as the newuserapprove plug-in
just want to be able to add a few extra fields to the initial registration AND add a captcha, i clear off about 200 spam attacks a week
