Need a questionnaire

Hello isaacotokpa,
Please create the questionnaire page first. Once that is done, next you need to show up the questionnaire page in your home page as a “Menu”. For that please go to your wordpress.com home page. In the left hand side panel you will get one option “Customize”. Under that you will get one option “Menus”. Under that you will get options like “Primary Menu”, “Social Menu” etc. and also “Menu Location”. Once you will expand “Primary Menu”, you will get a option “Add Items”. Clicking on “Add Items” you will see that your questionnaire page is listed under that. Click on the + sign next to it. Save the changes and next important thing is to assign menu location.
Select the menu location from available option like “Social Menu”, “Footer Menu” etc. As per your location selection the questionnaire page will show up on the top section or footer section of your home page.
