WordPress has a guide that details all the steps necessary to move from WordPress.com to a Self-Hosted WordPress installation on another host, including details about finding a new host and how to install WordPress on your new host.

https://move.wordpress.com/If you already have your new host chosen and WordPress up and running, you can start at the Export/Import step of the guide:

One thing that is important to note – Do *not* delete your site or its content from WordPress.com until the new site has been completely imported at your new host. The import process pulls the images over from your current wordpress.com site, so you need to keep the site up until that process is complete. There is more detail on this at the link above.
