Need ideas for email refferals – any pros ?

I hope to eventually monetize my blog. I intend to list business affiliates on my blog. Instead of displaying their email address i would like to have my website visitors contact them through my site, using it as a portal. My rational for this is, if my affilaites see that these referrals are coming as a result of my blog then they will see the value of being listed on my blog site as an affiliate. So I am trying to figure out the best way for my affiliates know that these referrals came from my my site.
Ideally i thought of an individual contact form for each affiliate. The form turns into an email to the affilaite that reminds that “This contact was brought to you by http://www.mywebblog.com” I know there must be numerous ways to accomplish what I want. Can anyone help me with ideas on how to best do this?

Oh, sorry. It’s http://tripquiver.wordpress.com/ However I have only just started building it beginning with a collection of cantacts I have amassed in the surf travel industry. You won’t see very much yet. I was hoping to find the best method to send refferals before I begin adding in the links to my affiliates.
